New Patients
If you would like to register as a patient with the practice, please complete the necessary forms. Once we have your completed forms & ID it can take up to 72 hours to get your details on the system.
The Merse Medical Practice is based within the Knoll Health Centre in Duns. If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill.
Our Practice area includes Duns, Chirnside & Berwickshire area. Please ask Reception for more information. If you move out-with the practice area you will be asked to register with a different GP surgery.You will need to complete a GP registration form and a new patient questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. (These are done by telephone)
How do I Register?
1: View/ Download / Collect Forms
If you would like register with the practice, you can either complete the online form, download and complete form manually or pick up a hard copy from reception.
2: Complete Forms
New patients are required to complete the following forms.

3: Return/send to the Practice
All applications must be returned to the practice reception complete and it is always helpful to have one form of valid and current identification such as a passport or drivers license. If this is not possible it will not delay your application.
